THE PURPOSE OF THIS AMENDMENT 0001 TO RFQ 36C25924Q0122 IS TO ANSWER VENDOR QUESTIONS, POST UPDATED SOW DATED 12.18 AND TO EXTEND RFQ SUBMISSION OF QUOTES DUE DATE AS INDICATED BELOW.

SUBMISSION OF QUOTES DEADLINE IS HEREBY EXTENDED TO DECEMBER 21, 2023 AT 16:00 MT.
Q1. Pedestal table - is this intended to be a dining table? Desired base and table top shape - round or square?
A1. Yes, will be used for dining. Square is preferred.
Q2. Game top table - round/cylinder or X base? Square or round top?
A2. Either base. Square preferred.
Q3. Armless chair - do you want this to be the same model as the arm chair?
A3. No, the VA is only wants chairs with arms.
Q4. Wall shelf - two shelves or four shelves? Will these be required to bolt to the wall? If so, what type of walls: cinder block, dry wall, other?
A4. Two shelves- The VA plans to hang them to the wall Some cinder block some drywall.
Q5. what is the intended purpose for the stool? We have a few options to provide. We have weigh down options that can be used as stools or a stool that is only down only.
A5. This is for a foot stool. Needs to be heavily weighted.
Q6. Delivery/installation/ballast: please confirm if you require ballasting before shipping and ballasting services on-site. Please confirm that Maintenance personnel at OKC VA can receive the chairs and put them in their respective areas at your facility. Is a lift gate required, or any other shipping requirements?
A6. If the furniture does not already meet weight requirements and will need to be filled with sand, furniture will need to be delivered already ballastered and assembled ready for use. Maintenance personnel at OKC VA can receive the chairs and put them in their respective areas at your facility. We have a dock for delivery.
Q7. Please clarify the installation that will be needed for the furniture on the request.
A7. No installation needed. We will receive the items and install things. However, the furniture will need to be delivered already weighted, ballastered and assembled ready for use. The contractor is responsible for filling with sand. All pricing is FOB Destination.
Q8. In the SOW, DT and Seating descriptions, it lists i. Drop Ship but in the requirements of each volume, it says Volume 2: (pricing)
price quote with separate line for labor/installation please clarify if installation is required or if it is just a drop ship.
A8. NO LABOR or INSTALLATION DROP SHIP ONLY unless items need to be ballastered or assembled.
Q9. In section PC2, Patient Chair, it says a preferred characteristic is h. Bariatric chair, and Glider available in the same line. Refer to the Patient Bariatric Chair for additional information But there is no glider on the solicitation. Is this something that is needed?
A9. IF IT IS NOT LISTED IT IS NOT NEEDED.
Q10. For the dining tables, DT1 appears to be the Forte table from Norix but the requirement says the tables are to come fully assembled. These come with the top separate from the base. If we are to include installation, it s no problem, but if it s a drop-ship, these do not come assembled.
A10. UPDATED verbiage in SOW. Items need to be assembled and ready for use.
Q11. For the game table, DT2, it appears to also be a Forte from Norix but theirs is a cross base. Pineapple has one that is all rotomolded with the cruciform base they were requesting but it doesn t have the laminate top.

Q12. Seating PC1: it says it has to have bleach cleanable upholstery, be rotationally molded and be available in wood or metal. There is no chair that is rotationally molded AND available in wood or metal. Because the color states Twilight , please clarify chair if it should be upholstered. The Norix Forte armchair is available with the twilight top and black base and can have an upholstered seat and back insert and meets the weight requirement. Should it be Upholstered or just rotomold?

Q13. Seating PC2: same as above but in addition, there is no bariatric chair available from any Norix line. All chairs can accommodate 750 lbs or more but they do not have a rotomold bariatric size. Pineapple has some love seat sizes that can be bariatric size but they are larger and narrower than the specs (over 49 w). They also would not be available in Twilight (that is unique to Norix). There is a matching chair but these are definitely not as Loungy as the Norix chairs. Both chairs say they prefer a glider in the same series. There is no rotomold glider and there is not a glider on the solicitation.
A13. The purpose of these chairs are to go up against a table. A love seat would be too wide and needs to be wider than the standard chair to accommodate the bariatric patient needs. No glider is required.
Q14. Line 5 of the price schedule lists a wall shelf but there is no description in the SOW. Should it be one shelf, 2, 3 or 4?
A14. This has been ADDED. Please include description in SOW
Q15. If installation is required, does anything need to be floor attached? I know the wall shelves need to be wall attached. Do they require pick-proof caulking?
A15. Items should arrive ballastered and assembled. No additional installation is needed beyond this.
