Performance Work Statement (PWS)
Annual Medical Gas Inspection & Certification Service
GENERAL:

This project is for a contractor to provide medical gas inspection and certification services in accordance with NFPA 99, Health Care Facilities Code, current edition as well as the terms and conditions stated within this SOW annually for the Oklahoma City VA Health Care System, Oklahoma City, Oklahoma.

The contractor is fully responsible for verifying and validating all information contained within this statement of work. Contractor must fully evaluate the condition of the existing structures and verify the current condition of the project site.

All measurements/quantities provided in this document are approximate and are to be used for reference only. The contractor will be responsible for taking on-site measurements and verifying all quantities and distances.

All materials/fasteners/etc. must be new and provided by the contractor unless otherwise specified.

The contractor shall abide by all Safety, Security and Infection Control Policies and Procedures of the Oklahoma City VA HCS.

VA facilities are tobacco free. No smoking is permitted on VA property. The contractor is responsible for their subcontractor s compliance with this policy.

All work must conform to, and comply with, the latest edition/version of ALL applicable VA, federal, state, and local laws, rules, regulations, codes, and standards.

BACKGROUND:

This facility is approximately 1,000,000 square feet with 10 floors containing the equipment and outlets to be inspected and certified. All work will be performed within the facility.

The medical gas system is comprised of:

STATEMENT OF WORK:

The contractor shall provide supervision, manpower, and materials to conduct a medical gas inspection and certification services in accordance with NFPA 99, Health Care Facilities Code.

Contractor shall ensure each requirement listed in this PWS is satisfied and that constraints are honored and respected. If the contractor has any conflicts or challenges with these, they should consult the COR or CO.
Contractor shall furnish all contract oversight, management, technically trained personnel, labor, equipment, tools, materials, supplies, insurances, and transportations necessary to provide scheduled Medical Gas Inspection and Certification Services
If the contractor detects that any Government equipment or component being inspected under the contract is in need of reconditioning, repair, replacement (or disposal), details shall be submitted in writing to the COR. The Contractor shall not undertake or perform any reconditioning, repair, or replacement of equipment without prior authorization from the COR.
The contractor shall provide the COR a copy of NITC Medical Gas Verifier 6030 licenses, required for person/people performing the services in this SOW prior to the conducting inspections. Either a paper or electronic version which may be emailed must be provided within:
15 business days of contract award and
15 business days after any contract option year awards
The inspector shall carry the license with him/her during the entire duration of the inspection.
The Contractor must provide the COR with certification attesting that all gauging and measurement equipment used to check and calibrate the subject items of equipment has been calibrated either according to the manufacturer s specifications or within the last twelve months against a standard that is traceable to the National Bureau of Standards. Such certification shall be provided with the service report submitted.
The contractor shall provide the COR written confirmation that the Contractor has adequate insurance protection against negligent work and other forms of liability. The nature and level of protection must be acceptable to this VA. Either a paper version or emailed version must be provided to the COR within:
15 business days of contract award and
15 business days after any contract option year awards
The contractor shall immediately report to the COR on all safety or operational related deficiencies during the inspection.
The Contractor shall provide the COR with a Service Report detailing all tests performed, all findings, and all recommendations. The Service Report is to be signed and dated by designated Contractor POC and given to the COR within two weeks of performing the Inspection Service. The Service Report is to be submitted by email to the COR in addition to a printed copy.
Contractor shall provide a statement describing their policy for charging of any additional fees due to rescheduling if equipment is not available at the time of the approved scheduled service call.
Contractor shall contact the COR upon completion of inspection

LIMITATIONS OF SUBCONTRACTING: The Limitations of Subcontracting may or may not apply to a resulting solicitation from this Market Research. Vendors are advised to familiarize themselves with Federal Acquisition Regulation parts 19.505(a)(2) and 19.505(b)(1) as it pertains to subcontracting limitations. Vendors are also advised to familiarize themselves with 13 CFR 125.6 and the penalties described in this regulation.
